Day Break Adult Day Health Care

Located at the Bloss Memorial Center, Day Break provides an organized day program for the elderly and other adults with physical and mental impairments.

We strive to preserve the dignity of our clients and enhance their quality of life by providing a caring, invigorating, and therapeutic environment that is both enjoyable and stimulating. 

Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of members. Our team provides community-based coordination of services and comprehensive care management.
