Enhanced Care Management (ECM)

What We Offer

Enhanced Care Management (ECM) provides additional services at no cost to Medi-Cal recipients with complex needs that make it difficult for them to improve their health. 

If you qualify, ECM offers six services that can help you with your health and well-being.

These extra services are part of your current Health Net Medi-Cal plan. The Medi-Cal services you get now will not be taken away. You can still see the same doctors, but now you can get extra help.


If you qualify, you can choose to get ECM services. Plus, you can stop the services at any time by calling Health Net.

An ECM Manager will Help You:

  • Find doctors, schedule appointments, and accompany you to visits
  • Manage prescriptions
  • Arrange transportation to doctor and pharmacy visits
  • Apply for community-based programs based on individual needs, including housing and food
  • Set up follow-up care after dismissal from the hospital

Who We Serve:

  • Adults/Families Experiencing Homelessness
  • Adults at Risk for Avoidable Hospital/ER Utilization
  • Adults with Serious Mental Health and/or Substance Use Disorder Needs
  • Individuals Transitioning from Incarceration
  • Adults with Intellectual or Developmental Disabilities
  • Pregnant or Postpartum Adults
  • Adults Living in the Community and At-Risk for Long Term Care Institutionalization

ECM Qualifications:

  • High Risk for Institutionalization (overdose and/or suicide)
  • Uses Crisis Services, ER, Urgent Care, or Inpatient as a sole Source of Care
  • Two or more ED visits or two or more Hospitalizations due to Serious Mental Issues (SMI) or Substance Use Disorders (SUD) in the past 12 months
  • Pregnant/Postpartum Women
  • Children/Youth High Utilizers or with Serious Emotional Disturbance (SED)

Community Support Service

A new menu of services to comprehensively address the most complex challenges affecting the health of members, such as homelessness, unsafe housing, food insecurity, and other social needs. 


ECB and Community Support are separate initiatives, and some Medi-Cal beneficiaries will qualify only for ECM, or only for Community Support.

  • Post-Hospitalization Housing (short-term)
  • Housing Tenancy
  • Recuperative Care
  • Sobering Centers
  • Medically Supported Meals

Schedule an Appointment

Visit our location and bring the application along with you to the appointment. Please complete it beforehand if possible.

What to Bring to your Appointment

  • Some form of accepted identification 
  • Social Security Card/Number
  • Proof of insurance 
  • Application Form 
  • Cal-Fresh (if applicable) 

Approval from Primary Care Provider

  • Participant will need a current negative TB result 
  • Signed authorization from PCP

Wait for Approval

We will contact you if we need further documentation or information. It may take up to three weeks to be approved through the insurance and begin services.
